Under state law the Coroner of the county of death must be notified of all cremations of individuals who died in Marshall County.  Funeral Homes or other agencies who will be performing the cremation must submit a notification prior to preceding with the cremation process.  ONLY FUNERAL HOMES OR OTHER CREMATION AGENCIES MAY USE THIS FORM.   The easiest and quickest way to submit a notification is using the Electronic Cremation Notification below, however, click the below PDF link to access a printable form.

dEATH NOTIFICATION

IF YOU NEED TO REPORT A DEATH CALL 911.  This notification form is only for healthcare facilities, hospice organizations, or other agencies required under laws or regulations to report deaths to the Coroner.  This is not intended to report suspicious deaths, deaths reported under this system must have a certifying healthcare professional.  Notification requester's must be an agent of a healthcare organization.  The easiest and quickest way to submit a notification is using the Electronic Death Notification below, however, click the below PDF link to access a printable form.
